Título: EFFECT OF PORPHYROMONAS GINGIVALIS LPS ON PLACENTAL HISTOPATHOLOGY IN GESTATIONAL DIABETES

Resumo: Objective: The interaction between periodontal bacterial virulence factors (LPS) and gestational diabetes (GDM), and its impact on the maternal-placental unit, remains poorly understood. The objective was to evaluate the histopathological changes in placental explants from healthy and GDM pregnancies exposed to Pg-LPS and hyperglycemia.

Methods:Chorionic villi explants from 15 women(5 GDM, 10 healthy) were collected post-cesarean section and cultured under four conditions: normoglycemia(5 mM), hyperglycemia(25 mM), normoglycemia + Pg-LPS(1 µg/mL), and hyperglycemia + Pg-LPS. Tissues were processed and stained with hematoxylin-eosin to assess fibrinoid necrosis, vascular changes, and structural alterations.

Results:Under basal conditions, GDM placentas exhibited greater fibrinoid necrosis than healthy controls (P= 0.0038). In healthy placentas, Pg-LPS plus hyperglycemia increased fibrinoid necrosis (P= 0.0211) and vascular changes (P= 0.0333). GDM placentas showed a blunted vascular response (P= 0.0503). No significant changes were observed in general morphology.

Conclusion:Hyperglycemia and Pg-LPS may exacerbate placental injury in healthy pregnancies. These suggest that controlling periodontal inflammation could protect the placenta in normal pregnancies. Placentas with GDM could activate compensatory mechanisms that warrant further study. These findings lay the foundation for future research, using in vivo models, to evaluate alterations induced by the pathogenic mechanisms of periodontal bacteria in the maternal-placental unit.
