Título: Aggressive Late Recurrence of Mandibular Rhabdomyosarcoma: A Case Report

Resumo: A 14-year-old female was diagnosed with mandibular rhabdomyosarcoma, presenting with preauricular pain, lower left lip numbness, and a 13 mm lytic lesion on the mandibular ramus. She underwent chemotherapy (Ifosfamide, Vincristine, Doxorubicin) and radiotherapy (54 Gy); however, disease progression was observed, leading to surgical resection followed by an additional 15 Gy of radiation for residual disease. Ten years later, she developed chin numbness and a nodular lesion. MRI revealed an infiltrative mass with mandibular erosion and C2 vertebral involvement, confirmed as metastasis. She underwent further chemotherapy (Carboplatin, Vinorelbine, Temsirolimus) and re-irradiation (50 Gy) for recurrence. Progressive disease required additional systemic therapies (Gemcitabine, Docetaxel, Temsirolimus, Nivolumab, Bevacizumab, Pembrolizumab). Severe treatment-related toxicities ensued, and the patient died from disease progression and respiratory failure. This case underscores the risk of late recurrence and aggressive progression in head and neck rhabdomyosarcoma, emphasizing the importance of long-term follow-up and tailored multimodal therapy.
