Resumo: In situ oral mucosal melanomas are extremely rare tumors, and their etiology remains a subject of discussion in the literature. Due to their uncommon presentation and often asymptomatic nature, diagnosis can be challenging. This case involves a 78-year-old male melanoderm patient diagnosed in 2022. The diagnosis was prompted by a clinical examination, which revealed a non-exophytic, asymptomatic lesion extending from the lateral incisor to the right maxillary tuberosity. Further investigation included computed tomography, histopathological analysis, and immunohistochemistry. Histological findings showed melanocytic proliferation with focal atypia restricted to the squamous mucosa, confirming the diagnosis of in situ oral mucosal melanoma. The treatment consisted of a right maxillectomy followed by radiotherapy, with the aid of an intraoral stent throughout the radiotherapy period. The patient underwent maxillofacial prosthetic rehabilitation at all stages of treatment to provide comfort, maintain nutritional status, and ensure an adequate quality of life.
