Resumo: Granulomatous foreign body reaction is an uncommon delayed adverse reaction to the use of dermal fillers for orofacial aesthetic purposes. A 66-year-old female patient presented large painless cord-shaped nodules of softened consistency and normal color on the lower lip mucosa with six months of evolution. Based on the clinical hypothesis of double lip, an excisional biopsy was performed. Histological analysis revealed oral mucosa covered by normal epithelium. The subjacent fibrous connective tissue presented chronic granulomatous inflammation, mostly composed of macrophages showing intracytoplasmic microdroplets suggestive of filling material compatible with silicone, a permanent filler prohibited by the FDA for cosmetic use. Immunohistochemical study confirmed the macrophages-mediated inflammation by extensive positivity for CD68. Scanning electron microscopy demonstrated a honeycomb appearance in the connective tissue, corresponding to the negative images of the silicone material. Based on these aspects, a granulomatous foreign body reaction to silicone was established. The patient remains in recent follow-up.
