Título: ORTHOKERATINIZED ODONTOGENIC CYST: A DIAGNOSIS AMIDST INITIAL SUSPICION OF DENTIGEROUS CYST AND ODONTOGENIC KERATOCYST

Resumo: A 52-year-old female patient underwent a routine panoramic radiograph, which revealed a well-defined, unilocular radiolucent lesion with a radiopaque halo in the posterior right mandibular region, associated with an impacted tooth. The initial radiographic diagnosis included dentigerous cyst and odontogenic keratocyst. Clinically asymptomatic, the patient presented with vestibular cortical expansion suggestive of cystic capsule involvement. An incisional biopsy was performed, and the specimen was fixed in 10% formalin and submitted for histopathological analysis. Microscopic evaluation revealed a lining epithelium with a thick orthokeratinized layer, confirming the diagnosis of an orthokeratinized odontogenic cyst. This case highlights the importance of correlating clinical, radiographic, and histological findings in the diagnosis of gnathic bone lesions. Accurate differentiation is essential, as prognosis and treatment vary significantly among these entities. A multidisciplinary approach is crucial to ensure appropriate therapeutic decisions and optimal patient outcomes.
