Nome do Apresentador: Carolina Pereira da SILVA

Categoria do Trabalho: Painel de caso clínico (PCC)

Área Temática: Patologia Oral

Resumo: A 35-year-old female patient presented to the oral diagnosis service with a painful lesion on the buccal mucosa and attached gingiva, reporting symptom worsening during periods of stress. She denied cutaneous involvement. Intraoral examination revealed bilateral whitish lesions on the buccal mucosa, with or without associated vesicles and bullae, leading to superficial, painful erosions. Desquamative gingivitis was also noted. The clinical picture was suggestive of a vesiculobullous lesion of undetermined origin, with white areas displaying a lichenoid appearance. An incisional biopsy was performed. Histopathological analysis showed hyperplastic epithelium with basal cell degeneration and saw-tooth-shaped rete ridges, along with a dense, band-like lymphocytic infiltrate in the lamina propria, confirming the diagnosis of Bullous Lichen Planus. Treatment with clobetasol 0.05% for 15 days was prescribed, and the patient was referred to a dermatologist. She remains under clinical follow-up for five months, with significant symptom improvement and occasional mild recurrences.
