Resumo: A female patient, 27 years old, was referred to the Oral and Maxillofacial Surgery and Traumatology service due to facial swelling, reporting paresthesia in the mental region with a two-year evolution. On extraoral examination, swelling was observed in the buccal region, with a firm consistency, asymptomatic on palpation, and without any fluctuation point. In the intraoral examination, a lesion with a color similar to normal mucosa was noted, with a firm consistency and obliteration of the right mandibular vestibular sulcus. Facial computed tomography revealed a well-defined hyperdense image in the mandible, with a rupture of the cortical bone. Based on these characteristics, the central diagnostic hypothesis was ossifying fibroma. A partial mandibular resection was performed, and histopathological examination showed bone tissue with hypercellular fibrous marrow, cemented calcifications, and peripheral bone trabeculae, confirming the diagnosis. The patient remains under clinical follow-up, with no signs of recurrence for seven months.
