Título: Microinvasive Squamous Cell Carcinoma: Diagnosed at the Vermilion-Skin Junction of the Lower Lip

Resumo: A 62-year-old white female was referred from a Primary Health Unit for evaluation of an ulcerated lesion on the lower lip. Clinical examination revealed brownish macules on the face and arms, and a history of chronic occupational sun exposure during youth. At the vermilion-skin junction on the right side of the lower lip, a brownish, asymptomatic, rough-surfaced plaque measuring approximately 1 cm and with a two-year history was observed. Differential diagnoses included actinic cheilitis, exfoliative cheilitis, and oral lichen planus. An incisional biopsy of the vermilion zone was performed, and histopathological analysis confirmed microinvasive squamous cell carcinoma. This case underscores the importance of dental professionals in the early detection of lip cancer, particularly in transitional areas. Chronic sun exposure remains the primary risk factor. Preventive measures should include daily application of sunscreen (SPF >30) on the skin and lips, as well as regular clinical follow-up.
