Resumo: PANDAS is a autoimmune disorder that is characterized by the sudden onset of obsessive-compulsive or tic disorder symptoms following group A streptococcus infection, typically diagnosed in pediatric and prepubertal population. We present a case of an 11-year-old boy with multiple persistent oral inflammatory lesions that has been evaluated and treated by several specialists without success. He had no other comorbidities, and his past medical history was a throat infection a month before the oral lesions began, and anxiety and tic disorder that were not previously common in his behavior. The evaluated of psychologist raised the diagnostic hypothesis of PANDAS. Instead of considering the oral lesions as possible pathogenetic mechanism of an immune nature, we consider them as originated by the bodys response to the presence of bacterial antigens through the formation of specific antibodies. Treatment of PANDAS is diverse, and clinical consensus regarding optimal treatment strategy is lacking.
