Resumo: Mycoplasma pneumoniae is a cause of community acquired pneumonia, mostly associated with dermatological manifestations especially with mucosal involvement and targetoid cutaneous lesions. It was considered among the spectrum of erythema multiforme but recently, the condition is increasingly recognized as a separate entity called "mycoplasma induced rash and mucositis" (MIRM) with distinct epidemiological, clinical and histological features making it different from drug-induced Stevens-Johnson syndrome, toxic epidermal necrosis and erythema multiforme. Herein, we report the case of two patients with acute Mycoplasma pneumoniae respiratory tract infection presenting severe oral mucositis lesions in accordance with MIRM. For diagnosis, in addition to clinical evaluation, confirmatory testing was performed using IgM/IgG Mycoplasma antibody testing. Although there are no standardized treatment guidelines for patients suffering from MIRM, supportive management is the mainstay of treatment. In these cases, the supportive care with photobiomodulation and antibiotic therapy with azithromycin were essential for the best outcome for the patients.
