Resumo: Carcinoma ex-pleomorphic adenoma is an epithelial and/or myoepithelial neoplasm that arises in association with a primary or recurrent pleomorphic adenoma (PA). This study reports a 79-year-old male patient who presented in 2017 with a nodular, floating lesion on the right hard palate. A thin-needle aspiration was performed, which suggested PA. Then, an incisional biopsy confirmed the diagnosis of PA. In 2024, the patient returned reporting a progressively enlarging mass in the oropharyngeal region, and an excisional biopsy was performed. Microscopic examination revealed a glandular neoplasm characterized by a proliferation of myoepithelial cells arranged in solid blocks, which occasionally lost cohesion, resulting in irregular spaces. There were areas exhibiting squamous metaplasia, with individual cells vacuolated and prominent nucleoli. Immunohistochemical markers showed positivity for cytokeratin, p63 protein, SOX10, calponin, S100, confirming an extensively invasive carcinoma ex-pleomorphic adenoma of high-grade myoepithelial cells. The patient was referred for oncological intervention, and remains under follow-up.
