Título: CLINICO-DEMOGRAPHIC ANALYSIS OF ORAL PIGMENTED LESIONS: A 20-YEAR RETROSPECTIVE CROSS-SECTIONAL STUDY

Resumo: This cross-sectional study evaluated the clinical and demographic characteristics of individuals treated at an Oral Medicine service between 2005 and 2025. Through the clinical image review of a digital database, 176 patients presented pigmented lesions of the oral mucosa, revealing an annual average of 8.8 consultations with these conditions, and their medical records were analyzed. A higher prevalence was observed among females (70%) and individuals with fair skin (51%). Smokers and former smokers accounted for 17% and 22.1% of the sample, respectively. The most commonly affected intraoral sites were the buccal mucosa (38%) and gingiva (34%). Clinically, the lesions were characterized by brownish coloration, irregular or diffuse shape, and a smooth surface. Exogenous pigmentation, such as tobacco melanosis, and post-inflammatory pigmentation in patients with oral lichen planus were the most commonly assigned diagnoses. Interestingly, in 60% of cases, the pigmented lesion was diagnosed secondarily, meaning it was not related to the patients chief complaint. Proper review, analysis, and documentation of oral pigmented lesions are essential for distinguishing them from less common conditions with similar clinical features. Moreover, this process contributes to improved patient management and guidance, ensuring appropriate therapeutic approaches when necessary.
