Resumo: Lymphoepithelial carcinoma (LEC) is a rare malignant epithelial tumor which can occasionally arise in salivary glands, particularly the parotid, characterized by undifferentiated epithelial cells within a dense lymphoid stroma. This report describes the case of an 80-year-old female who presented with a rapidly enlarging and painful mass in the parotid gland region. Surgical excision of the lesion was performed. Histopathological analysis revealed large nests of anaplastic epithelial cells embedded in dense lymphoid stroma, associated with areas of desmoplasia, perineural invasion, positive surgical margins, and involvement of regional lymph nodes. Based on these findings, a diagnosis of lymphoepithelial carcinoma was established, classified as pT4a pN2b. The specimen was submitted for immunohistochemical analysis to better define the prognostic profile. The patient is currently awaiting further surgical intervention and remains under regular oncologic follow-up.
