Resumo: Myoepithelial carcinoma (MECA) is a rare invasive malignant neoplasm of the salivary glands, that can originate de novo or from a pre-existing pleomorphic adenoma (PA). This case reports a 46-year-old male patient who presented a submandibular nodule 9 years ago (2017), that were diagnosed with a PA with atypia and underwent excision of the left submandibular gland. Histological examination revealed a malignant neoplasm of ectodermal origin, infiltrative growth of spindle-shaped and vacuolated cells with clear cytoplasm, nuclei featuring clefts and occasionally distinct nucleoli. The cells were arranged in irregular cords and nests, exhibiting hyperchromatism. Immunohistochemical markers established the diagnosis of Myoepithelial Carcinoma, and the patient was referred for radiotherapy and chemotherapy until 2021. In 2024, he experienced a recurrence, with infiltrative growth of the lesion, aphasia, pain while chewing solid foods, and mandibular pain. A pelveglossomandibulectomy was performed, reaffirming the previous diagnosis. The patient remains in paliative treatment.
