Resumo: A 54-year-old woman presented with an asymptomatic, exophytic lesion on the ventral surface of the tongue, suggestive of benign squamous papilloma (BSP). The lesion, measuring 0.7 cm, had been present for two months. Clinical and microscopic analysisconfirmed BSP, with rare koilocyte-like changes and acantholytic cells observed. Immunohistochemical staining revealed strong positivity for HSV-1, and weaker positivity for HSV-2, while HPV was negative. A diagnosis of HSV-infected BSP was made, a rareoccurrence. The patient had no relevant medical history, and no similar lesions were found elsewhere. She underwent excisional biopsy, and after one year of follow-up, there was no recurrence. This case highlights the unusual interaction between HSV and BSP, with HSV-1 identified as the likely cause of the lesion, and emphasizes the need for careful differential diagnosis in such lesions.
