Resumo: Two women, a 55-year-old melanoderma and a 75-year-old leucoderma, presented pain in the oral mucosa due to erythematous and ulcerated lesions, diagnosed as hypersensitivity reaction and oral lichenoid lesion, respectively. During the diagnostic process, the intraoral examination revealed diffuse melanotic pigmentation on the palate, buccal, labial mucosa, and lips in one patient, and on the lower lip in the other. Extraoral examination showed melanonychia on their fingernails. Diagnoses were racial pigmentation and Laugier-Hunziker syndrome. The patients were informed and remain under follow-up. Oral melanotic pigmentation may have endogenous, exogenous, or syndromic causes. Though more common on the gingiva, racial pigmentation can affect any oral area and also the nails. Laugier-Hunziker syndrome is diagnosed after excluding other potential causes. This highlights the importance of a thorough clinical evaluation, considering medical history, harmful habits, and both intraoral and extraoral findings, to ensure an accurate diagnosis.
