Resumo: Pemphigus vulgaris (PV) is an autoimmune disease characterized by IgG autoantibodiesagainst desmogleins 1 and/or 3, leading to acantholysis and the formation of intraepidermalblisters. In neonates, it occurs through passive transplacental transfer of these antibodies andis generally transient. Although self-limited, neonatal pemphigus requires early diagnosis todifferentiate it from other severe dermatoses. We describe a case of PV in a newborn. Themother presented with blistering lesions on the tongue, oral mucosa, breasts, and skin, whilethe newborn exhibited ulcerated lesions in the genital, axillary, and posterior neck regions.The diagnosis was confirmed by incisional biopsy and histopathology, which revealedsuprabasal cleavage, a "tombstone" pattern, and a lymphocytic inflammatory infiltrate.Prednisolone was administered at a dose of 100 mg/day for 14 days, with progressiveimprovement and the newborn, had a good improvement. Conclusion: This case reinforces theimportance of early diagnosis and clinical surveillance in neonates exposed to maternalautoantibodies.
