Título: PLASMABLASTIC LYMPHOMA IN THE ORAL CAVITY: CASE REPORT WITH INITIAL HIV DIAGNOSIS AND AGGRESSIVE PROGRESSION

Resumo: A 54-year-old male patient presented with bilateral nodular, reddish, and whitish vegetative lesions on the buccal mucosa and alveolar ridges. Preoperative tests revealed positive HIV serology, leading to referral to the infectious disease team. Initial diagnostic hypotheses included lymphoma and Kaposi's sarcoma. An incisional biopsy followed by immunohistochemistry showed a mesenchymal neoplasm with strong Ki67 positivity (95% proliferative index in B lymphocytes), confirming a diagnosis of plasmablastic lymphoma. The patient began chemotherapy and antiretroviral therapy (ART), showing significant improvement in oral lesions after four months. However, the disease became refractory to chemotherapy, and the patient died after seven months of treatment. Plasmablastic lymphoma is a rare entity, accounting for approximately 2% of non-Hodgkin lymphomas, predominantly associated with HIV and Epstein-Barr virus. This case highlights the importance of early clinical recognition of oral manifestations, biopsy with immunohistochemistry, and close monitoring due to the aggressive nature and poor prognosis of the disease.
