Resumo: A 41-year-old female patient presented for evaluation due to the presence of a vegetative nodule located on the alveolar ridge in the region of teeth 33 and 34, which had previously been extracted due to pain and mobility. An initial incisional biopsy suggested a malignant neoplasm, but histopathological review was inconclusive, leading to a second biopsy. The final diagnosis, confirmed by immunohistochemistry, was spindle cell embryonal rhabdomyosarcoma (ERMS). The patient underwent mandibulectomy with cervical dissection, microvascular flap reconstruction, and radiotherapy. Tumor recurrence required additional surgery. ERMS is a rare malignant tumor originating from rhabdomyoblasts, most commonly affecting the head and neck region, and is uncommon in adults, accounting for approximately 3% of childhood cancers. It is a highly aggressive neoplasm requiring a multidisciplinary approach. This case highlights the role of the dental surgeon in early diagnosis and therapeutic support, contributing significantly to clinical management and the patients quality of life.
