Título: THE KEY ROLE OF THE DENTAL SURGEON IN DIAGNOSING MYELOID SARCOMA AND ADVOCATING FOR ADVANCED THERAPIES: A CASE REPORT

Resumo: A 76-year-old female with acute myeloid leukemia under azacitidine therapy presented with right-sided gingival enlargement extending from anterior to molar regions. Histopathological analysis revealed atypical myeloid cells proliferation with nuclear pleomorphism, high mitotic activity, and necrosis. Immunohistochemistry was positive for CD45, myeloperoxidase, and CD68, with a Ki-67 proliferation index of 80%, confirming myeloid sarcoma. Due to disease progression despite azacitidine monotherapy, a multidisciplinary team initiated a formal insurance appeal process to secure treatment coverage for venetoclax therapy, a high-cost drug with evidence of improved survival and remission rates. The treatment regimen was successfully modified to azacitidine-venetoclax combination therapy. Currently, the patient remains under treatment but exhibits thrombocytopenia-related sequelae. This case highlights the critical role of the dental surgeon in diagnosing complex systemic conditions with oral manifestations and ensuring patient access to advanced therapies through legal and interdisciplinary efforts.
