Título: PHOTODYNAMIC THERAPY AS AN ADJUNCTIVE TREATMENT FOR RECURRENT ORAL ERYTHEMA MULTIFORME: A CASE REPORT

Resumo: A 30-year-old male patient reported recurrent, painful oral ulcers unresponsive to systemic corticosteroid therapy, impairing speech and eating. Intraoral examination revealed multiple shallow ulcers, surrounded by erythema, throughout the oral mucosa. White removable plaques, suggestive of opportunistic candidiasis, were also observed. The biopsy was inconclusive, showing nonspecific chronic inflammation with ulceration. Serological tests were positive for herpes simplex and showed low basal cortisol levels, indicating adrenal suppression. The patient was diagnosed with recurrent erythema multiforme, likely herpes-related. Weekly photodynamic therapy (aPDT) was initiated alongside acyclovir prescription, corticosteroid discontinuation, and referral for medical evaluation. After two aPDT sessions, lesion remission was observed, and clinical follow-up was maintained. Monitoring corticosteroid use is essential in patients with immune-mediated inflammatory diseases to prevent self-medication or other misuse, and the risks of adrenal suppression. Low-level laser therapies are valuable tools in these scenarios, both aPDT and photobiomodulation, depending on the presence of concomitant infection.
