Resumo: Cemento-ossifying fibroma (COF) is a benign intraosseous odontogenic neoplasm that may occasionally cause local destruction. It is characterized by the replacement of normal bone with cellular connective tissue containing varying amounts of mineralized material. A 41-year-old male patient underwent cone-beam computed tomography (CBCT) for surgical planning of the extraction of tooth 38. The imaging revealed a well-defined, hypodense apical lesion associated with the mandibular third molar, measuring approximately 10 mm in diameter. The primary differential diagnoses included chronic apical periodontitis and a benign fibro-osseous lesion. Following surgical excision, histopathological analysis demonstrated fibrous and cellular connective tissue with moderate vascularity and deposition of both osteoid and cementoid material. Based on these findings, a definitive diagnosis of COF was established. The patient remains under clinical and radiographic follow-up, with no evidence of recurrence to date.
