Resumo: Deep suppurative cheilitis is an extremely rare form of glandular cheilitis. This condition usually affects minor salivary glands of middle-aged males, and its etiology remains unclear. Hereinafter, we present a case of a 56-year-old male with a one-week history of painful swelling in the lower lip. During anamnesis, the patient reported working in chicken manure production. Physical examination revealed multiple ulcerated areas, deep suppurated tracts, and edema involving the lower lip. Under hypothesis of deep fungal infection and glandular cheilitis an incisional biopsy was performed. Microscopic analysis showed dilated excretory ducts, some with metaplasia, intense periductal acute and chronic inflammation, and degeneration of the minor salivary gland parenchyma. PAS and Grocott staining were negative for fungal presence. The final diagnosis was deep suppurative glandular cheilitis. The patient was successfully treated with systemic antibiotic therapy and advised to avoid direct sun exposure. No recurrence was observed after one year of follow-up.
