Resumo: Cystadenomas are rare benign neoplasms representing 1-4% of all salivary gland neoplasms. Adult and elderly females are frequently affected. About 45% of cases involve the parotid, while intraoral lesions mimic mucocele, with lip and buccal mucosa being the most common sites. By microscopy, it presents as an unilocular (20%) or multicystic lesion, frequently lined by papillary projections. A 56-year-old woman presented with a nodular lesion on the right buccal mucosa, suggesting a reactive or benign salivary gland lesion. After excisional biopsy, microscopy revealed a multicystic glandular lesion with mucinous and oncocytic features. Low-grade MEC was initially suggested, however, immunohistochemistry showed positivity for p40/p63 only in basal cells, while α-SMA and calponin were negative. p53 exhibited a wild-type pattern, and Ki-67 evidenced rare cells. Cystadenoma with mucinous and oncocytic metaplasia should be included in the differential diagnosis of low-grade MEC, especially unicystic ones, with p40/p63 expression pattern being relevant.
