Resumo: Bisphosphonates are widely used for treating bone metabolism disorders and bone metastases. However, their adverse effects, particularly bisphosphonate-related osteonecrosis of the jaw (BRONJ), have gained attention due to increasing reports and clinical challenges.This case report presents a 61-year-old female with metastatic breast cancer using monthly zoledronic acid (Zometa®), who developed mandibular pain and swelling. Imaging revealed osteolysis and necrotic bone in the mandible.The patient underwent surgical treatment involving removal of necrotic bone, infected implants, and placement of a reconstruction plate.Postoperative management included low-level laser therapy, hyperbaric oxygen therapy, and multidisciplinary care. Despite some recurrence of bone exposure, additional outpatient interventions and laser sessions led to complete healing.The case highlights the complexity of treating BRONJ in oncologic patients, especially those with compromised immunity. We emphasize the importance of preventive care, early diagnosis, surgical management in advanced stages, and close follow-up with a multidisciplinary team to improve outcomes and quality of life.
