Resumo: Cementoblastoma is a rare benign neoplasm derived from odontogenic ectomesenchyme, characterised by osteocementum-like tissue formation attached to the tooth root. It exhibits expansive growth and carries a risk of recurrence. Radiographically, it presents as a radiopaque mass fused to the root, surrounded by a radiolucent halo. Diagnosis is based on strict clinical, imaginological and histopathological correlation. By microscopy, cementoblastoma reveals thick trabeculae of osteocementum-like tissue lined by prominent cementoblasts in strict continuity with the root cementum, and a thin layer of fibrous tissue demarcating the lesion at the periphery. We report a 39-year-old female patient who presented an irregular hyperdense lesion involving the root surface of the impacted tooth #48. The clinical and imaginological diagnosis favored hypercementosis, however, benign fibro-osseous lesion could not be completely excluded. After surgical removal of the lesion, histopathological analysis confirmed the diagnosis of cementoblastoma. After 2-year follow-up, the patient is well, without alterations.
