Resumo: A 34-year-old male patient was referred for evaluation of an asymptomatic lesion in the left inferior vestibular sulcus, with approximately eight months of evolution. Clinically, a whitish area of the oral mucosa was observed. Imaging studies included computed tomography, which revealed localized bone resorption in the chin region. An initial hypothesis of endodontic origin involving tooth 31 was ruled out after further evaluation. An excisional biopsy of the lesion was performed and submitted for histopathological analysis. Microscopically, dense connective tissue was observed, interspersed with granulation tissue composed of lymphocytes, plasma cells, young fibroblasts, and neovascular structures. Multinucleated giant cells of the foreign body type and macrophages were seen surrounding basophilic exogenous material. The diagnosis was consistent with a foreign body granuloma. Correlation with the patient's clinical history revealed prior hyaluronic acid filler injection in the region, suggesting delayed adverse reaction. Management included surgical removal of the material and clinical follow-up.
