Resumo: White sponge nevus is a rare hereditary epithelial disorder characterized by mutations in keratins 4 and 13, resulting in a thickened, spongy oral mucosa. A 58-year-old male Caucasian patient presented with bilateral white lesions on the lateral tongue, which exhibited a thickened and corrugated surface, causing aesthetic discomfort. Clinical differential diagnoses included leukoplakia, lichen planus, and candidiasis. Following an incisional biopsy, microscopic examination revealed epithelial thickening with parakeratosis, acanthosis and dyskeratotic cells with paranuclear eosinophilic condensation. A diagnosis of white sponge nevus was confirmed. Treatment with topical tetracycline was administered, resulting in regression of the lesions. The patient is under ongoing clinical follow-up, with no recurrence observed.
