Resumo: Keratoacanthoma (KA) is a benign, self-limiting epithelial neoplasm associated with excessive exposure to ultraviolet radiation, mainly affecting older individuals. A 59-year-old man presented with an asymptomatic nodule with a verrucous surface, located in the right lower lip, measuring approximately 0.4 cm, with a 6-month evolution. In his past medical history, the patient had undergone radiotherapy and chemotherapy for the treatment of sinonasal carcinoma, 15 years ago. Based on the clinical presentation, an excisional biopsy was performed, and the specimen was submitted for histopathological analysis, which revealed hyperplastic epithelium with a crateriform center containing keratin and no cellular atypia. Thus, the diagnosis of KA was established. The patient has been under regular clinical follow-up for 11 years ago, with no evidence of recurrence. Due to its histopathological features being very similar to well-differentiated squamous cell carcinoma, the diagnosis of KA can be challenging for clinicians and oral pathologists.
