Resumo: Familial florid cemento-osseous dysplasia (FFCOD) is a benign fibro-osseous lesion, classified as a variant of cemento-osseous dysplasia in latest WHO classification. An 18-year-old Caucasian female was referred for assessment of radiopaque maxillary and mixed mandibular bilateral lesions, associated with impacted teeth and enlarged alveolar ridges. She reported successive infections in the right maxillary bone, where dental extractions, a biopsy, and histopathological examination were performed, confirming florid cemento-osseous dysplasia (FCOD). Later that year, her 13-year-old sister presented for evaluation of a radiolucent area between the roots of a healthy lower molar, and delayed tooth eruption. Over a 10-year follow-up, the primary lesion evolved, and additional lesions on maxillary and mandibular bones appeared, showing clinical and radiographic characteristics of FCOD, and confirming its familial pattern. These cases highlight the distinct behavior of FFCOD compared to other variants, including its hereditary nature, early onset, and association with impacted teeth and bone expansion.
