Resumo: Squamous cell carcinoma (SCC) is the most common malignant neoplasm of the oral cavity, typically presenting as an ulcer with indurated borders. However, atypical clinical manifestations may hinder early diagnosis. This study reports two cases with unusual clinical presentations. The first case involves a 63-year-old female patient presenting with a small rough-surfaced lesion surrounding the superior canine. The initial clinical differential diagnoses included granulation tissue, papilloma, and condyloma. Histopathological examination revealed microinvasive SCC. The second case concerns an 83-year-old male patient with an exophytic nodular lesion, sessile, irregular, and whitish appearance, located at the midline of the dorsal tongue, initially diagnosed as atypical median rhomboid glossitis. Histopathological analysis led to diagnosis of SCC. These cases underscore the importance of a thorough clinical approach when evaluating atypical oral lesions, highlighting histopathological examination as an indispensable tool for definitive diagnosis and appropriate therapeutic planning.
