Resumo: Myofibroma is a rare benign mesenchymal tumor arising from the abnormal proliferation of myofibroblasts, with intraosseous involvement of the jaws being uncommon. We present the case of a 16-year-old female patient who developed pain and swelling in the right mandibular angle nine months after the extraction of the lower right third molar. Intraoral examination revealed a nodular mass with a fibrous consistency. Computed tomography (CT) imaging demonstrated a hypodense lesion with indistinct borders, cortical expansion, and areas of cortical perforation. Histopathological analysis of the surgically excised lesion revealed a proliferation of spindle-shaped and round neoplastic cells arranged in a fascicular pattern within a collagenized fibrous stroma. The neoplastic cells exhibited small, rounded nuclei and eosinophilic cytoplasm. Immunohistochemical staining demonstrated strong positivity for α-SMA. Based on these findings, a diagnosis of myofibroma was established. The patient remains under follow-up, with no signs of recurrence after six months.
