Título: Primary Botryomycosis of the Tongue: A Tumor-Like Diagnostic Challenge

Resumo: Botryomycosis is a supurative granulomatous bacterial infection, extremely rare in oral sites, that can mimic other infectious or neoplastic conditions. We report a case of a 60-year-old male with an ssymptomatic, firm, well-defined, pink to reddish nodular lesion on the dorsal tongue, evolving over one year, causing voice changes and limited tongue mobility. The clinical diagnosis was neurofibroma or schwannoma. Incisional biopsy revealed non-caseating chronic granulomatous inflammation with focal microabscesses. Special stains Wade, PAS, Grocott, and Warthin-Starry were negative, but Giemsa and Brown-Brenn staining showed Gram-positive cocci colonies amid irradiated amorphous material, consistent with the Hoeppli-Splendore phenomenon. Clinicopathological correlation confirmed botryomycosis. The patient received antibiotic therapy for five weeks and remains recurrence-free. Due to its rarity and potential for misdiagnosis, botryomycosis should be considered in the differential diagnosis of tongue nodules, particularly in patients with prior trauma or infection. Thorough investigation is crucial to ensure accurate diagnosis and appropriate management.
