Título: Severe Pemphigus Vulgaris Induced by Radiotherapy: A Challenging Case of Extensive Mucocutaneous Ulcerations

Resumo: A 59-year-old male patient, a former smoker and alcohol consumer, presented with a non-healing ulcer on the floor of mouth. Clinical examination revealed a diffuse, expansive ulcerative lesion. An incisional biopsy and histopatholgical analysis led to diagnosis of squamous cell carcinoma. The patient underwent surgical resection with cervical lymph node dissection, followed by 33 sessions of radiotherapy (total dose: 6600 cGy) and adjuvant chemotherapy with cisplatin. One month after completing radiotherapy, the patient developed painful ulcerations with thick crusting on the lips and malar region. The lesions rapidly progressed, affecting the neck, back, and chin, resulting in severe epithelial disruption. Due to worsening condition, the patient required hospitalization for 40 days and enteral nutrition via a nasoenteric tube. After skin biopsy diagnosis of pemphigus vulgaris was established. High-dose prednisone was initiated, leading to gradual lesion regression. The patient remains under follow-up, with no recurrence in the past three months.
