Resumo: Mesenchymal chondrosarcoma (MCS) is a high-grade, biphasic, malignant cartilaginous tumor that predominantly affects the craniofacial region of young adults. A 24-year-old man was referred for evaluation upon an incidental radiographic finding of a well-defined unilocular mixed radiolucent-radiopaque lesion associated with the right maxillary third molar. Under the clinical impression of a dentigerous cyst, an excisional biopsy was performed under local anesthesia. Microscopic analysis revealed a diffuse neoplasm composed of primitive undifferentiated round cells arranged in a perivascular hemangiopericytoma-like pattern, admixed with scarce islands of well-differentiated hyaline cartilage. The tumor cells expressed diffuse positivity for CD99, SOX9, NKX3-1, and D2-40, while S100 expression was restricted to the hyaline cartilage component. The cell proliferation index measured by Ki-67 was 5%. The final diagnosis of MCS was established. The patient was subsequently referred to oncologic treatment. MCS represents a rare malignancy that may mimic odontogenic lesions when occurring in the jaws.
