Resumo: A 25-year-old female patient was referred for a dental evaluation of a left facial swelling. During clinical examination, facial asymmetry was noted in the middle third of the left side. The intraoral clinical examination revealed a mass in the left maxillary molar region. Computed tomography showed a destructive expansive lesion in the left maxilla, involving the lateral and posterior walls of the maxillary sinus. An incisional biopsy was conducted under local anesthesia, and the tissue sample was sent for histological analysis, which indicated the proliferation of round cells. Immunohistochemical tests were negative for AE1/3, S100, synaptophysin, chromogranin, CD20, CD3, MYOD1, desmin, TLE1, SATB2, NKX2.2, and positive for CD99 and cyclin D1. These findings led to the diagnosis of high-grade small round cell sarcoma. The patient was referred to the oncological surgery service, where she underwent hemimaxillectomy and radiotherapy. Unfortunately, the patient died five months after the initiation of treatment.
