Título: FACIAL ASSYMETRY IN A YOUNG PATIENT LEADING TO A SURPRISING DIAGNOSIS: PEARLS AND PITFALLS RELATED TO PLEOMORPHIC LIPOMA

Resumo: We report a case admitted in surgerys outpatient care with a cheek swelling with facial asymmetry and pain in a 17 years old female patient. Clinically the lesion manifested as a tumoral mass fixed in left buccal mucosa. Computed tomography scan revealed hypodense lesion in upper jaw. A white-colored gelatinous consistence lesion was obtained through incisional biopsy. Microscopic sections revealed infiltrative pleomorphic adipose tissue with some areas of myxoid, chondroid or multilobulated lipomatous appearance, with peripheral encapsulation, well vascularized, eosinophilic material with a mucinous juxtacapsular appearance, mucinous degeneration type, presence of lipoblasts with giant and hyperchromatic nuclei, trinucleated floret cells and focal bone metaplasia. Immunohistochemistry revealed CD34 +, S100 +, CDK4 +, Vimentin +, focally positive SMA, PanCK -, MDM2 - and Ki-67 on 2% of cells, confirming diagnosis of pleomorphic lipoma. Lesion was later surgically removed. The patients under 5 months post-diagnosis follow-up without recurrence or symptoms.
