Resumo: Pseudomonas aeruginosa is a rare oral pathogen, typically linked to immunosuppression and severe systemic conditions. Here, we present a 70-year-old female patient with aplastic anemia who was admitted with febrile neutropenia and intense oral pain. Oral evaluation revealed edema, erythematous mucosa, and ulcerated lesions on the bilateral buccal mucosa and lateral tongue. A clinical scraping of the palatal lesion was performed, and exfoliative cytology revealed the presence of Pseudomonas infection in the oral lesion. The patient underwent antimicrobial photodynamic therapy (aPDT) during hospitalization until medical discharge. Additional exams included chest CT (suggestive of fungal infection), galactomannan, abdominal ultrasound, polysomnography, PET-CT, and lung biopsy. Treatment included antibiotics (Meronem, Targocid), antifungals (fluconazole, voriconazole), and corticosteroids (Solumedrol). The patient died due to septicemia caused by Candida. This case highlights an uncommon oral infection by P. aeruginosa, highlighting the importance of dental evaluation in immunocompromised individuals.
