Resumo: Fibrous dysplasia is a developmental anomaly that affects bone maturation and remodeling, replacing the medullary and cortical bone with disorganized and immature fibro-osseous matrix, making the patient susceptible to bone pain, skeletal deformities and pathological fractures. It can manifest itself in several ways, and can cause loss of vision, hearing and respiratory difficulty. This study aims to report the case of a patient with McCune-Albright Syndrome and deforming craniofacial fibrous dysplasia. Physical examination revealed significant facial asymmetry, associated with exacerbated maxillary growth, accompanied by several maxillomandibular alterations that triggered respiratory impairment. Radiographic examination revealed diffuse craniofacial bone thickening with expansive lytic lesions in the bone marrow. It was decided to remove the excess and perform bone plasty in the maxilla through a Weber-Ferguson approach and in the mandible through a modified Newman approach. The case evolved with improvement in the respiratory condition and reduction in facial asymmetry.
