Resumo: A 61-year-old female patient with a history of mental disorders, gastroesophageal reflux disease, burning sensation of the tongue, and recurrent aphthous ulcers was under multiple medications. The patient presented with a firm, sessile, verrucous, painless, pink nodule with whitish spots on the left buccal mucosa. The lesion measured approximately 5 mm and was located near the linea alba. The patient was completely edentulous and wore ill-fitting mucosa-supported dentures. After surgical excision, histopathological examination revealed acanthotic stratified squamous epithelium and fibrous connective tissue hyperplasia associated with chronic inflammatory infiltrate, consistent with inflammatory fibrous hyperplasia. Fungal hyphae penetrating vertically through the epithelium were observed and confirmed using Grocotts methenamine silver and periodic acid-Schiff stains, indicating fungal infection by Candida spp. The final diagnosis was inflammatory fibrous hyperplasia associated with fungal infection. The patient received adjunctive antifungal therapy and remains under clinical follow-up.
