Resumo: This case series presents four atypical herpetic infection cases, highlighting diagnostic challenges and the evolving clinical spectrum of herpes simplex virus type 1 (HSV-1)-related lesions. The patients included two males and two females, aged 40 to 76 years. Three cases involved persistent painful ulcers on the tongue border, while one presented as recurrent whitish papules on the upper and lower lip semimucosa. Clinical hypotheses included sexually transmitted infections for the tongue ulcers and mucocele for the lip lesions. One patient had a history of rheumatoid arthritis and melanoma on immunotherapy, while the others had no significant medical conditions. Histopathological analysis revealed ulcers, intraepithelial vesicles, and cytopathic changes such as ballooning, nuclear inclusions, molding, and marginalization. Immunohistochemical study confirmed HSV-1 infection. Treatments included biopsy, photobiomodulation, and antivirals. These findings emphasize the need for thorough evaluation of atypical herpetic lesions to ensure accurate diagnosis and appropriate management.
