Resumo: Nodular basal cell carcinoma (BCC) is a malignant neoplasm of the skin. The adenoid subtype is a rare variant with a histologic cribiform growth pattern. A 60-year-old male with a history of smoking and alcohol consumption presented to the hospitals dental service with a 2 cm blackened nodular asymptomatic lesion on the upper lip. An incisional biopsy revealed nodular masses of basaloid cells extending into the dermis in a cribiform pattern with dispersed pigmentation. The tumor showed deep invasion into the muscular tissue. The diagnosis confirmed nodular basal cell carcinoma, adenoid subtype, with deep tissue infiltration. The patient was referred to an oncologist for evaluation and treatment. Early recognition and biopsy of suspicious lip lesions are crucial, as prompt diagnosis and referral optimize patient outcomes, especially in high-risk patients.
