Resumo: Gingival squamous cell carcinoma (SCC) is a rare malignancy that can clinically mimic benign inflammatory and reactive lesions. We report the case of a 74-year-old woman presenting with a painless, exophytic, erythematous mass on the anterior mandibular gingiva and alveolar ridge, initially diagnosed clinically as a pyogenic granuloma. Panoramic radiography revealed an area of extensive bone loss. Incisional biopsy showed an atypical proliferation of epithelial cells arranged in nests and cords with squamous differentiation. Immunohistochemistry was positive for pan-cytokeratin, 34βE12, p63, and Ki-67 (30%). The diagnosis was gingival SCC. The patient was referred to the oncology department for complementary surgery and adjuvant radiotherapy. The initial misclassification of the lesion as a reactive condition underscores the need for heightened clinical suspicion and prompt histopathological evaluation. Given the aggressive nature of gingival SCC and its potential for local recurrence, early detection, appropriate treatment, and long-term follow-up remain essential for improving patient outcomes.
