Título: INTRAORAL SALIVARY DUCT CARCINOMA WITH PROMINENT CLEAR CYTOPLASMIC VACUOLE AND STROMAL SCLEROSIS WITH ELASTOFIBROMATOSIS

Resumo: Salivary duct carcinoma (SDC) is a rare and aggressive malignancy, predominantly affecting the parotid glands of adult males, with intraoral involvement being uncommon. A 66-year-old male was referred presenting a large tumor mass on the left buccal over several months, initially diagnosed as high-grade mucoepidermoid carcinoma. Upon re-evaluation, microscopic analysis revealed extensive stromal sclerosis with foci of elastosis occupying about 85% of the tumor area, alongside discrete tumor nests with pleomorphic and hyperchromatic nuclei and numerous clear cytoplasmic vacuoles. Comedo-type necrosis and apocrine differentiation were not evident. Immunohistochemistry showed strong positivity for CK7, GCDFP-15, GATA3, and androgen receptor. HER2 IHC indicated a 2+ score, and HER2 FISH evidenced significant amplification in over 50% of cells. The p53 IHC pattern was abnormal ("null"), and the Ki-67 proliferative index was 30%. The diagnosis was consistent with sclerosing SDC. After 16 months of follow-up, the patient remains well, without recurrences.
