Resumo: A 76-year-old female patient presented with a nodular lesion on the left lower lip mucosa, measuring 1.5 cm, with a smooth surface, well- circumscribed, normal color, firm and mobile on palpation, with a five-year history of progression. An ultrasound of the region was requested, revealing an oval, hypoechoic formation with defined edges. Mesenchymal neoplasia was considered as a diagnostic hypothesis. An excisional biopsy was performed, and histopathological analysis showed an encapsulated neoplasia composed of spindle-shaped Schwann cells distributed in a homogeneous pattern with a myxoid background (Antoni B), along with in palisaded areas forming Verocay bodies (Antoni A). The final diagnosis was Schwannoma. After seven months of clinical follow-up, no recurrence has been observed. It is important to include schwannoma in the differential diagnosis of lower lip lesions.
