Resumo: Systemic mastocytosis (SM) is a rare disease characterized by the abnormal proliferation of mast cells in organs beyond the skin, such as bone marrow, gastrointestinal tract, including the liver and spleen. Eosinophilia, in turn, is the increase in eosinophils in the peripheral blood, above 3%-5% of leukocytes. The aim of this study is to report the case of a 44-year-old male with multiple painful ulcers, with irregular borders, whitish beds, in various regions of the oral mucosa. The patient had a previous diagnosis of systemic mastocytosis and severe eosinophilia. The main clinical hypothesis was recurrent aphthous ulceration. Incisional biopsy revealed an edematous and vascularized stroma with infiltration of mast cells, atypical mononuclear cells, lymphocytes, and few eosinophils. The treatment consisted of low-level laser therapy sessions, topical corticosteroids, and, at times, systemic corticosteroids. The patient is under follow-up and has shown significant clinical improvement of the ulcerated lesions.
