Resumo: Sialadenoma papilliferum (SP) is a rare, benign neoplasm of the salivary glands, more commonly observed in elderly males and primarily located on the palate, lip, and buccal mucosa. Clinically, it presents as a slow-growing, exophytic lesion, which may be mistaken for other oral lesions, as squamous papilloma and verrucous hyperplasia. Diagnosis is established through histopathological examination, which reveals papillary projections lined by squamous epithelium and underlying ductal proliferation. Two types, classic and oncocytic, have been described. We report an 80-year-old female patient presenting with a rounded, asymptomatic nodular lesion on the hard palate, measuring about 10 x 9 x 4 mm. After excisional biopsy, microscopy confirmed classic type of SP. By immunohistochemistry, pan-CK AE1/AE3, CK7, EMA, p40/p63/D2-40/α-SMA (basal cells), and BRAF V600E (VE1) were positive. Moreover, p53 showed wild-type pattern and Ki-67 highlighted rare cells. In the current case, the immunohistochemical profile of basal cells in consistent with myoepithelial differentiation.
