Resumo: Amyopathic dermatomyositis (ADM) is a rare autoimmune disease affecting only the skin, without muscle involvement, and may be associated with Sjögren's syndrome (SS). A 45-year-old female patient diagnosed with ADM was referred to the Stomatology Clinic with xerostomia and xerophthalmia. Extraoral exam revealed erythroderma, V-shaped rash on the neck, shawl sign, joint papules, and cuticles with telangiectasia. Intraorally, the mucosa was poorly lubricated, with non-removable white plaques and asymptomatic erosive areas on the buccal mucosa, bilaterally. SS was suspected, and a labial salivary gland biopsy showed a focus score >1 focus/4mm². Serological testing and sialometry (< 0.1 ml/min) confirmed the diagnosis. An incisional biopsy of the buccal mucosa was performed. Histopathology showed basal layer degeneration, mild dysplasia, and an eosinophilic band adjacent to the basement membrane, with hyperemic vessels and lymphocytic infiltrate in the lamina propria. Oral manifestations of ADM are poorly understood and resemble those of other autoimmune diseases.
