Resumo: Schwannomas are benign nerve sheath tumors arising from differentiated Schwann cells, frequently affecting the upper limbs, followed by head and neck, notably the tongue. Without sex predilection, adults are often involved, 90% occurring spontaneously. By microscopy, schwannoma variants include ancient, cellular, epithelioid, microcystic/reticular, neuroblastoma-like, and plexiform. Moreover, some cases display cystic changes, variably associated with ancient variant. Intraoral ancient or cystic variants of schwannoma are rarely reported. A 54-year-old female presented with an asymptomatic, indurated lingual mass one year ago. After excisional biopsy, the lesion appeared multicystic and well-defined, covered by pseudocapsule, containing numerous cells with marked nuclear atypia and loss of Antoni A tissue. Immunohistochemistry showed positivity for vimentin, S100, SOX10, and CD56. Ki-67 highlighted scarce cells but not the atypical cells, while p53 showed the wild-type pattern. To the best of our knowledge, intraoral ancient and cystic schwannoma, such as presented here, has not been previously reported.
