Resumo: Actinomycosis is a rare, chronic bacterial infection that can mimic malignant or fungal diseases, particularly in immunosuppressed patients. A 33-year-old female presented with painful fibrinopurulent lesions on the hard palate and upper lip, impairing oral intake, with 15 days evolution time. Her medical history revealed type I diabetes for over 25 years, which was complicated by pregnancy and hypertension, leading to end-stage renal disease and, consequently, a combined kidney-pancreas transplant four years prior to the current presentation. An incisional biopsy was performed, and the histopathological analysis showed typical features of actinomycosis. The patient was treated conservatively with benzathine benzylpenicillin (IM/two weeks), surgical debridement, and a 14-day course of amoxicillin (oral, 8/8h), leading to complete resolution of the lesions after four weeks. No recurrence was observed after 1-year follow-up. This case underscores the importance of early diagnosis of actinomycosis, especially in immunocompromised patients, enabling effective surgical and antibiotic treatment.
